Here is an interesting Infographic from Vree Health demonstrating that we are actually making progress in readmission reduction for some common conditions.  It will be interesting to see how we fared in terms of cost savings overall.  In other words, how much of the savings from preventable readmissions have been offset by the cost of the programs implemented to focus on readmissions plus the additional costs of treating the patient (appropriately) in an alternative setting? One thing we know about improving the cost and quality of health care is that it is really really hard.
